At the hidradenitis instituted our philosophy involves active treatment of disease that has come back repeatedly despite antibiotic treatment and drainage procedures. Often this requires little more than a limited local excision of the affected area. Occasionally it requires a more extensive removal with reconstruction.
We favor one stage reconstruction without the use of skin grafts. We favor reconstruction and wound closure that does not affect areas outside the location of the hidradenitis. Whenever possible we perform excision and reconstruction that does not require either any additional surgical procedures or prolonged wound care to clear the treated area of hidradenitis and heal all incisions. Hidradenitis suppurativa is a chronic condition, and surgery treats the areas operated on rather than curing the disease; new lesions can still develop elsewhere.
